Obituary

AMY LYNN URCHICK (nee SMITH), age 41 of Portage, passed away Thursday, November 6, 2014. Amy was born in Valparaiso on April 12, 1973 to Dennis and Ruth Smith. She was a lifelong area resident and a 1991 graduate of Portage High School. Amy worked as a healthcare provider at Sterlin g House and for the past nine years at Porter Regional Hospital. She was a member of Nativity of Our Savior Catholic Church. Amy will be remembered as a hardworking woman and loving wife, mother, daughter and sister.
She is survived by her husband, Denny Urchick; 2 sons, Nathan and Nicholas Urchick; parents, Dennis and Ruth Smith; 4 sisters, Amanda Moore, Heather (David) Morris, Heidi (David ) Ewen, Ashley (Steven) Hojnacki; grandmother, Dorothy Smith; 6 nieces and nephews, Kaitlin, Dylan, Owen, Ella, David, and Andrew; father-in-law, Dennis Urchick, Sr.; mother-in-law, Patricia Urchick; godparents, George and Phyllis Seberger; many aunts and uncles.
Amy was preceded in death by her paternal grandfather, Fred Smith, Sr. and maternal grandparents, Leonard and Pauline Seberger.
A Mass of Christian Burial will take place Monday, November 10, 2014 at 10:00 a.m. at Nativity of Our Savior Catholic Church, 2949 Willowcreek Rd., Portage, IN, with Rev. Andrew Corona officiating. Burial will follow at McCool Cemetery. Visitation will be Sunday, November 9, 2014 from 3:00-7:00 p.m. at Rees Funeral Home, Olson Chapel, 5341 Central Ave., Portage, There will be additional visitation on Monday at the church from 9:15 a.m. until the time of mass.
