Obituary

HAZEL L. FIELDS, age 82, of DeMotte, passed away Thursday July 7, 2011 at home. Hazel was born July 28, 1928 in Black Mountain, Kentucky to Sailor and Lula (nee Williams) Lewis. She had been a longtime Northwest Indiana resident and was a member of the Church of God of Prophecy. Hazel is survived by her loving husband of 63 years, Hobart Fields and 2 daughters, Rosa (John P.) Sullivan of DeMotte and Rebecca (Raymond) Keller of Valparaiso, 2 grandchildren: Bryan (Royanna) Sullivan & Hollie Morris and 2 great grandchildren: Kaylee & Isabell, 2 brothers: Troy Lewis of Portage, Danny (Glenda) Lewis of Valparaiso, 2 sisters: Mary (Glen) Noe of Portage, Barbara (Chuck) Snyder of Florida, brother-in-law, Fred Bowles of Knox, numerous nieces & nephews. She was preceded in death by her parents: Sailor and Lula Lewis, siblings: Rosa Lewis, Homer Lewis, Louise Harkness and Janice Bowles. Visitation will be held Sunday, July 10, 2011 from 4:00 to 8:00 pm at Rees Funeral Home, Winfield Chapel, 10909 Randolph Street, Crown Point ( Winfield), IN 46307. Mrs. Fields will be taken to Church Monday morning at 10 am for visitation from 10:00 to 11:00 am. Funeral services for Hazel will be Monday, July 11, 2011 at The Church of God of Prophecy, 110 N. Jefferson St., Hebron, IN, beginning at 11:00 am with Pastor J. Elwood Adams officiating with her daughter, Rev. Rebecca Keller sharing the eulogy. Burial will follow at Hebron Cemetery.
