Maylon "Jack" Cook, age 81, of portage passed away Sunday December 14, 2014. Jack was born November 16, 1933 in Troy, Tennessee to the late Robert Maylon and Mary Ruth Cook. Jack retired from Calumet Flexicore in Hammond, Indiana as a maintenance foreman in 1995. Jack was a member of the Central Avenue Church of God in Portage.
Jack is survived by his loving wife of 19 years, Wilma Cook of Portage; two sons Jackie (Kim) Cook of Hanna and Barry Cook of Portage; a stepdaughter Anita (Dewey) Mayfield of Portage; eight grandchildren; 13 great grandchildren; and his beloved dog Pugie.
Jack was preceded in death by his first wife Martha, his son Michael Cook and his parents.
Funeral services will be 10:00 am Thursday, December 18, 2014 at the Central Avenue Church of God 5120 Central Avenue Portage, Indiana 46368 with Rev. Jerry P. Martin officiating. Burial will follow in Calvary Cemetery. Visitation will be from 2:00 to 8:00 pm on Wednesday, December 17, 2014 at Rees Funeral Home 600 W. Old Ridge Road in Hobart.
