Patricia "Poppy" Slavik

Patricia Ann “Poppy” Slavik, age 69, of Portage, passed away Monday, August 24, 2026. She was born on May 10, 1957, in Gary, Indiana, to the late John and Joyce Slavik. Patricia retired from St. Mary Medical Center. She enjoyed playing softball, bowling in leagues at Ray’s Lanes, and watching old movies. Patricia loved animals, especially her beloved cats, Rocky, Elliott, and Pepper.

She is survived by her companion, Michele Potts of Portage, IN; one brother, Michael Slavik of Lake Station, IN; one sister, Brenda (Terri Stasko) Slavik of Hobart, IN; longtime family friend, Sharon Oller of Roxanne, IL; and many beloved nieces and nephews.

In lieu of flowers, memorial donations in Patricia’s honor may be made to the Hobart Humane Society or the American Cancer Society.

Memorial services are Friday, September 18, 2026 at 1 p.m. at Rees Funeral Home, 600 W. Old Ridge Road, Hobart.  There will be a procession to Calvary Cemetery for burial of cremains immediately following the memorial service.  Memorial visitation is just prior to the service from 11 a.m. to 1 p.m. at Rees Funeral Home, Hobart.
