Robert Frank Polak, Jr.
Robert Frank Polak, Jr., age 52, of Portage, passed away Thursday, September 24, 2020 at St. Mary Medical Center in Hobart. He was born in Hammond, Indiana on October 6, 1967 to Robert and Linda (nee Keilman) Polak.
Robert is survived by his father, Robert Polak, Sr. of Hobart; daughters, Ashley Polak of Warren, Ohio, Samantha Polak of Warren, Ohio; brothers, Michael (Pam) Polak of Valparaiso, Christopher (Amanda) Polak of Hobart; and sister, Tammy Stanley of Lake Station.
He was preceded in death by his mother, Linda Polak.
Cremation has been entrusted to Rees Funeral Home, Hobart Chapel. 219-942-2109.
